Affidavits

What is an Affidavit?

  1. A written declaration made under oath.
  2. Being under oath means the statement is sworn to be true.

When is an Affidavit required?

  1. To request a copy of a lost document.
  2. To confirm your address if the utility bill is not in your name.
  3. To grant permission for someone to act on your behalf (e.g., renewing a license disc).
  4. To provide verification for certain applications.
  5. To de-register your vehicle (only through the Police Department).

To have your affidavit verified:

  1. Documents supporting your statement.
  2. Your identity document.
  3. The signed affidavit.
